The rules for entering your medical records are simple.
You can use a form that’s already on file with the government.
You’ll also need to fill in your name, address, phone number and email address, as well as your date of birth.
You have to submit the records within 30 days.
If you have a doctor’s note or other medical record that needs to be submitted, it’s a good idea to send it to the state’s Department of Health and Human Services.
The agency will review it and make the right decision.
In many cases, doctors and hospitals have a way to keep track of their records.
If you’re a resident of New York State, your records will be automatically sent to the Office of Medical Records at the State Hospital in Buffalo.
You don’t need to send them to a different state.
If they need to be kept confidential, you’ll need to request that the records be kept private.
